With the recent announcement from TD Garden of its new public safety guideline, it made us ask – where the heck did I put my vaccination card?
If you can’t find your card, you have a few options via the State of Massachusetts website –
- If you were vaccinated by your primary care provider, at a community health center, or local health department, contact your primary care provider.
- If you were vaccinated at a local health department or “pop-up” clinic: contact your primary care provider.
- If you were vaccinated at a Massachusetts mass vaccination site, you can contact your primary care provider or follow these instructions here.
- If you were vaccinated at a retail chain like CVS etc. you can follow these instructions.
The TD Garden requires that anyone 12 years and older must provide proof of full vaccination against COVID-19~ OR proof of a qualifying negative COVID-19 test* to gain entry. So if you’re heading to the upcoming Enrique Iglesias and Ricky Martin concert – get the vaccination card ready.
